Did you know that you can order repeat prescriptions, book appointments and view parts of your medical record online?

To register for this service please complete the application form available from reception at Audlem Medical Practice and bring photographic ID and proof of address. For example a passport and utility bill.

Using these documents we will generate the necessary user names that you need.
