How was your medical appointment?

Audlem Medical Practice is conducting a survey of patients' experience at their last appointment or call to the medical centre.

Printed versions of the questionnaire are available at Boots in Cheshire Street, the post office, the shop in Woore and at the medical practice.

There is also the option to complete online. The link is available on the Medical Practice website reached by clicking on the link below the photo.

The results of the survey will be discussed with the Patient Panel members at their next meeting in early November.
