New medical patient questionnaire

The Audlem Medical Practice is carrying out the PPG patient questionnaire again, which can be found here

We hope many people can complete the survey which provides the practice with very useful feedback on the services we offer.

There are eight questions in all and there is room for you to add comments. Your help will be much appreciated.
