Autism survey

East Cheshire LINk have been asked to support the Adult Autism Strategy. The questionnaire – please click on the link below the photo – is part of this strategy.

If you or a member of your family, or perhaps a friend or neighbour has autism, would you please pass this on and to tell or assist them to fill it in and send it back to us.

The date on the paper does state 31st October as the close, but if we can get it back as soon as possible – even if it is a week after the deadline – it will be included.
